A patient registration tool for dental practices that simplifies the form experience and satisfies specific practice needs with smart customization.
Overview
Patient FastTrack is a tool that is included in the Lighthouse 360 product. It allows dental practices to capture information from new patients by sending them a simple, mobile-friendly form to fill out prior to their first appointment. After releasing an MVP and measuring practice adoption, I was tasked with designing the next iteration of the feature with the goal of satisfying practice needs and ultimately increasing adoption.

How might we capture enough new patient information with Patient FastTrack so that practices do not have to distribute paper forms when patients come in for their appointment while maintaining a simple, user-friendly form experience for patients.
